Our disease prevention services include but are not limited to: Genetic Testing Services (pharmacogenomics & nutrigenomics), CDC diabetes prevention program (DPP), annual wellness visits (AWV), opioid addiction prevention program, smoking/tobacco cessation consultation service, women’s and men’s preventative health services that includes an extensive health and wellness assessment, biometric screening services and much more.

We deliver an evidence based and CDC approved 12 month diabetes prevention lifestyle program with statistically significant success rates. Participants are paired with highly trained and compassionate health coaches that utilize a CDC approved curriculum to educate and facilitate twenty-six group discussions between 5-20 participants. Participants lose 5-10% of their starting weight by the end of 12 months and reduce their diabetes risk by 58%. Classes cover a variety of health & wellness topics including meal planning, tips for eating away from home, and ways to increase daily physical activity. Participants meet face to face or by video conference to receive continuous support and encouragement from their coach and a small group of peers facing similar challenges.

Our clinical pharmacists collaboratively deliver a unique approach to annual wellness visits by incorporating their medication expertise during each visit. They are able to quickly identify, address and resolve any medication-related issue while also providing patients a better understanding of their medication therapy. With support from physicians and non-physician providers, our Pharmacists are able to provide personalized prevention plans for patients that take a comprehensive approach to improve health and preventing disease.

Our program involves Pharmacist partnership with patients and prescribers to prevent opioid addiction. We provide patient counseling services prior to the initiation of opioid therapy for risk assessment. High-risk patients are closely monitored through the prescription monitoring program to ensure the appropriate use of opioid therapy. High-risk patients also receive continued counseling, education, and recommendations to community support resources for addiction prevention. Our Pharmacist team provides recommendations to prescribers when addictive patterns are identified through the prescription monitoring program

Our nicotine cessation program is an 8 week program offered through weekly internet-based, telephonic or in-person group sessions. Our team of specialists provides motivational tips, advice, and educational resources that help participants overcome challenges to stop smoking, vaping, and stop other tobacco use. We also offer clinical Pharmacist consultation for incorporating individualized nicotine replacement therapies for added success.

Our service include an extensive health & wellness assessment and plan, biometric screening, immunization recommendation and referral along with preventative disease screening for depression, osteoporosis, alcohol abuse & other preventable conditions. We also provide customized wellness plans and packages related to women’s health. Our wellness plans and packages are tailored to address each individual’s specific health challenges and support the achievement of their health-related goals.
