YOUR PATH OUR PURPOSE
Waiver Support Coordination is a personalized service that advocates for individuals and helps them identify, develop, coordinate, and access the support they need—regardless of the funding source. Coordinators work closely with each person to ensure they receive the right services and can also empower individuals and their families to navigate these resources independently. Support may come from various sources, including the iBudget Waiver, Medicaid state plan services, third-party payers, and natural supports such as family and community. Coordinators also assist with accessing broader resources available through state, federal, and local government programs. Their goal is to help people succeed in every area of life—at home, at work, in school, and throughout their community.
