TEAM UP WITH PRIME CARE TO PREVENT DIABETES

Prime Care Health Solutions provides a Center for Disease Control and Prevention (CDC) approved diabetes prevention program which shows that by eating healthier, engaging in moderate physical activity, and losing just 7% of body weight, a person with pre-diabetes can prevent or delay the onset of type 2 diabetes.
Our Diabetes Prevention Program can help you help your patients. Based on research from the National Institutes of Health, programs like Prime Care’s Diabetes Prevention Program reduce new cases of type 2 diabetes by 58% overall and 71% in individuals over age 60.
The program is a year-long lifestyle change program that engages participants in 25 sessions throughout the year. With instruction and support from Lifestyle Coaches, participants learn how to incorporate healthier eating, moderate physical activity, and problem-solving and coping skills into their daily lives. The curriculum is CDC-approved and is part of the CDC-led National Diabetes Prevention Program.
The program focuses on small, measurable goals to give participants confidence they can make the necessary changes to reduce their risk for type 2 diabetes. The group support keeps participants motivated. You can feel confident that the patients you refer will spend a year surrounded by supportive people with common goals.

Why you should TEAM up – Today
The Centers for Disease Control and Prevention (CDC) and the American Medical Association (AMA) are sounding an alarm about pre-diabetes because a national effort—by everyone from physicians to employers to patients to community organizations—is required to prevent type 2 diabetes in the United States. In addition to focusing on the person with pre-diabetes or diabetes, we also must engage the systems and communities where people live, work and play. We can all TEAM up to prevent diabetes.

How you can help prevent Type-2 Diabetes
Healthcare professionals can help prevent type-2 diabetes by referring patients at risk for type 2 diabetes to our TEAM diabetes prevention program (DPP). You can be the link between your patients and this program for healthier living. Referring to a patient is simple. When you refer a patient, we will contact the patient and provide detailed information about the program and its benefits and assist with enrollment.

How Patients Can Qualify
There are two ways for patients to qualify. Patients can take a risk analysis or the result of recent lab results that identify diabetes risk can be submitted for patients to qualify for the program. If a patient has one of the following blood values in the last 12 months, they qualify for the program:
- HbA1c between 5.7% and 6.4%
- Fasting plasma glucose between 100 – 125 mg/dL
- A 2-hour plasma glucose level of 140-199 mg/dL or higher
